As a direct specialty care sleep clinic, we do not bill insurance (out of network). All new patients start with a Comprehensive Consultation. After initial consultation, you may join a monthly membership for ongoing management with visits and communication included as needed. See FAQs for more information.
Summary of Direct Payment Model at ReLACS Health
Comprehensive Consultation
$750 + $100
For patients new to ReLACS Health, you may schedule a Comprehensive Consultation for approximately 90 minutes through telemedicine. You must attest to being present in Michigan, Ohio, or Florida at the time of consultation. A $100 non-refundable chart preparation fee and signed Patient Agreement is required prior to consultation. Ongoing treatment and communication after consultation will require subsequent monthly membership. Otherwise, a detailed consultation report will be available to you and sent to your physician(s).
Meet & Greet
$35
For those who want more information about the practice or are considering whether to schedule an initial consultation, you can schedule a 20-minute video Meet & Greet with Dr. Berkowski; this is an informal visit and no medical specific management will take place. The fee is non-refundable and is due prior to the meeting.
Standard Membership
$150/month
For patients who require long-term management of sleep health, including advice, answering questions, behavioral modifications, medication or device management, coordination of care, etc. Membership begins after starting consultation. You can schedule telemedicine appointments as needed and communicate directly with your doctor through secure texting/SMS, portal messaging, or phone at your discretion. Payment for each month is due one month from date of initial consultation. Those completing a specialized management membership will revert to Standard Membership thereafter.
Severe Restless Legs Syndrome-Augmentation Management Program
$800/month for 5 months
After initial consultation, those with severe restless legs syndrome (RLS) symptoms, including augmentation from dopamine agonist medications or difficulty recovering, may enter into this unprecedented program. Dr. Berkowski has treated a few hundred individuals with augmentation in his career using a systematic, evidenced-based approach incorporating the most up to date treatments. Not only is he a national expert in RLS, he will be at your side to guide you through this problem and into recovery. After the Severe RLS Management Program has completed, you may automatically continue as a Standard Member or return to your regular sleep or primary care physician at your discretion.
Comprehensive Insomnia Management Program
Standard Membership + $185 per session
Based on initial consultation, an individual with chronic insomnia who is a good candidate for cognitive and behavioral therapy for insomnia (CBT-I) may join the Comprehensive Insomnia Management Program. This first-line, but hard to find, treatment involves generally 5—7 telemedicine or phone appointments over two months. All sleep-related conditions, if present, can be managed simultaneously with membership. Supplements, over-the-counter treatments, and prescription medications are not typically used for insomnia as these are not first-line treatments. After completion of CBT-I, you may elect to continue Standard Membership or may return to your regular sleep or primary care physician at your discretion.
Home Sleep Apnea Testing
$350
This test to diagnose or reassess the presence of obstructive sleep apnea (OSA) is conveniently delivered to and then picked up from your home by commercial shipping, and your sleep can be monitored from the comfort of your home bedroom. Traditional sleep centers may charge $700-$1200 or more for out-of-pocket payment for this. Consultation at ReLACS Health is not required if we receive an order from a licensed medical provider. Test must be performed in Michigan or Ohio. Click here for more details. ReLACS Health members receive discounted rate if testing is necessary during the course of membership.
Return Consultation
$450
For non-members who have been a patient at ReLACS Health in the last 15 months, you may schedule a Return Consultation of approximately 60 minutes via telemedicine. You must be present in Michigan, Ohio, or Florida at the time of consultation. You may choose to join ongoing membership thereafter.
Tranquility Membership
$75/month
Standard Members may request Tranquility Membership if they have had no significant changes to sleep health for at least three months, are clinically stable, but require ongoing medication supervision, prescriptions, and availability for personal health advice.
